Employee (m/f/d) in social services

part-time.

• Advice and support for patients and their relatives in social and psychosocial matters
• Mediation of offers of help and organization of aftercare and rehabilitation
• Collaboration with external service providers and authorities
• Support in case management and discharge planning
• Documentation and reporting

• A degree in social pedagogy or social work

• Completed training as a medical assistant with relevant experience
• Nursing training with additional qualification in case management
• Commercial training with experience in healthcare or further training in social services
• Legally required proof of immunization against measles
