We are looking for you to join our team in the Somatics department as
Social Services Employee (m/f/d)
at the earliest possible date.
Advice, care and support for our patients and/or their relatives in social law matters
Discharge management and individual coordination of post-hospital care for our patients
General administrative activities, maintaining contacts with authorities and outpatient and inpatient facilities
Arranging rehabilitation measures or further services, such as self-help groups
Initiation of and advice on legal guardianship, information on health care proxies and living wills
Advice on social legislation (care, pensions, health insurance, the Disability Discrimination Act, social assistance, etc.)
Completed studies in the field of social pedagogy/social work or completed vocational training as a health and nursing professional (m/f/d) with additional qualification in the field of case management/care transition
Experience in hospital social services or similar areas in the healthcare sector is desirable
High social competence as well as process-oriented, innovative, economic thinking and acting
Patient-oriented and cooperative working style
High level of communication skills, commitment and ability to work in a team, flexibility
Legally required proof of immunization against measles
