For our location at the Helios Clinic Bonn/Rhein-Sieg, we are looking for a full-time or part-time employee in social services (m/f/d) as soon as possible.

• Psychosocial counseling
• Assistance in the clarification and organisation of post-inpatient care (e.g. outpatient and inpatient care, provision of aids, initiation of rehabilitation measures (AHB))
• Advice on legal guardianship, powers of attorney and living wills
• Advice on social benefits (Severely Disabled Persons Act, long-term care insurance, etc.)
• Arranging contacts to offices, counselling centres and self-help groups

• A completed training in health and nursing with corresponding experience in discharge management
• Professional experience, preferably in social services or the care transition of a clinic
• Knowledge of social law issues according to SGB II, V, VI, XI, XII
• Strong communication skills and self-confidence
• Social skills and empathy
• Friendliness and open-mindedness towards patients and relatives
• Ability to collaborate in an interdisciplinary and multi-professional manner
• Good verbal and written expression
• Security in dealing with IT
• Proof of complete vaccination against measles
