full and part-time.

• Advice and support for patients and their relatives in social and psychosocial matters

• Referral to offers of help and organization of aftercare and rehabilitation
• Cooperation with external service providers and authorities
• Support in case management and discharge planning
• Documentation and reporting

• A degree in social pedagogy or social work is desirable

• Completed training as a medical assistant (m/f/d) with relevant experience
• Nursing training with additional qualification in case management desirable
• Commercial training with experience in healthcare or further training in social services
• Legally required proof of immunization against measles
