We are looking for a social services nurse (m/f/d) to start as soon as possible.

• Advice for patients and relatives regarding post-inpatient support needs
• Coordination of discharge planning
• Close cooperation with doctors, nursing staff and other professional groups involved
• Cooperation with health insurance companies, pension insurance providers, nursing services, nursing homes, rehabilitation clinics and all other aftercare institutions
• Processing the associated applications

• Completed three years of nursing training
• An independent and structured way of working
• Ability to work under pressure, flexibility, ability to work in a team and mobility
• Good communication skills and customer-oriented work
• You have the legally required proof of immunization against measles
