We are looking for the next possible date at the Munich Pasing location for a

Social Services Employee (m/f/d)

full-time or part-time.

• Planning and coordination of discharge management
• Networking with external providers of nursing, care and rehabilitation
• Assisting and advising patients in personal and financial emergencies
• Organizing outpatient care and advising patients as well as placing patients in short-term care, retirement homes and hospices
• Initiating rehabilitation as well as providing advice and submitting applications within the framework of social legislation
• Clarification of inpatient treatment costs for patients without health insurance

• Completed training as a registered health and nursing professional (m/f/d), health and pediatric nurse (m/f/d) or geriatric nurse (m/f/d) with several years of professional experience in the social sector or in various patient-related areas
• Good knowledge and experience in conducting conversations and counseling with patients and relatives
• A high level of commitment, a confident manner, empathy, very good time management and pedagogical skills round off your profile
• Good IT skills and knowledge of social legislation SGB V and SGB XI desirable
• A high degree of flexibility, open-mindedness and commitment as well as a willingness to work openly, collegially and in an interdisciplinary manner
• Legally required proof of immunization against measles
