For the Center for Social Counseling and Discharge Management at Helios Klinikum Schwelm, we are looking for a Social Services Employee (m/f/d) to start as soon as possible.

• Coordination of post-clinical care for patients as part of discharge management

• Advice on post-inpatient care and suitable care aids
• Communication with cost bearers, authorities, rehabilitation clinics and other institutions
• Cooperation with doctors and nursing services, among others
• Support with applications for, among other things, care degree classifications and administrative processes

• Completed three-year professional nursing training

• Comprehensive knowledge of care structures, assistance services and social insurance law
• High level of social and counseling skills as well as empathy in dealing with patients and their relatives
• Confident handling of documentation systems and organizational processes in the healthcare sector
• Legally required proof of immunization against measles

• Part-time employment (50%)
