For our location at Helios Klinikum Niederberg, we are looking for a social services employee (m/f/d) for our patient service center with a focus on hospital social services and discharge planning for patients as soon as possible.

• You will optimize the care of chronically ill patients or patients with complex medical conditions
• You will plan, coordinate, document and evaluate post-hospital care according to the patient's individual needs and the clinic's available resources
• You are the point of contact for patients from admission to discharge or transfer to other care systems
• You will also be involved in the development of treatment pathways
• Individual tasks include advising on care insurance, including applications, transfer to the home and thus networking with outpatient care services, outpatient hospice services and other therapists and aftercare providers, initiation of full inpatient care in nursing homes, residential groups or hospices as well as initiation of indication-specific or geriatric rehabilitation measures
• Furthermore, the area of responsibility includes identifying the need for aids, setting up care and advice on health care proxies and living wills, contact with cost bearers, authorities, rehabilitation clinics, self-help groups, counseling centers, inpatient facilities and outpatient follow-up care as well as in-clinic cooperation with the nursing service staff, the medical service and therapists from other professional groups

• Completed training as a healthcare and nursing professional (m/f/d) with several years of professional experience in various specialist departments
• Strong organizational and analytical skills, above-average commitment and a results- and goal-oriented way of working
• Comprehensive knowledge of care structures and support services and a professional and responsible ability to assess and select
• Experience in administrative work and PC skills
• A high level of social competence and sensitivity in dealing with patients and their relatives in line with the Helios service concept and quality of care
• A high level of counseling competence as well as process-oriented and rapid case handling
• Excellent communication and cooperation skills with all parties involved as well as a high degree of reliability, flexibility and resilience
• Legally required proof of immunization against measles
