To strengthen our team in the Patient Service Center, we are looking for a mini-job for a

Patient Service Center employee (m/f/d)

• Planning and organization of post-inpatient care for patients as part of the statutory discharge management process
• Advising patients and their relatives on psychosocial and socio-legal issues and problems
• Arranging and initiating medical rehabilitation measures
• Working independently
• Mediating, networking and administrative activities

• Completed studies in social work / social pedagogy or completed training as a health and nursing professional (m/f/d)
• Professional experience in the transfer of patients in hospital
• Good knowledge of social law and the common MS Office programs
• Very good communication skills, interest in dealing with people and excellent organizational skills
• High level of responsibility, commitment and reliability
