Helios Klinik Schleswig GmbH is looking for a part-time, permanent employee for Social Services (m/f/d) to start as soon as possible.

• Administrative tasks in the area of social services-coordination and organization of transfer and discharge planning
• Actively cover the need for care
• Maintaining contact and exchanging information with internal and external network partners
• Advice on and organization of AHB/rehab measures
• Participation in interdisciplinary case discussions/team meetings

• Completed training as a nursing specialist (m/f/d)
• Communication skills and above-average commitment as well as social skills
• Sensitivity in dealing with patients and caregivers in the special situation of a hospital stay
• Willingness for further education and training
• High flexibility and ability to work independently
• Legally required proof of immunization against measles
