• Planning and organization of outpatient nursing care
• Discharge planning in cooperation with patients and relatives
• Application for aids or care degrees
• Planning and organization of short-term care or nursing home places
• Communication with all institutions involved in the discharge process (e.g. health insurance companies, care insurance companies)

• Training as a social worker (m/f/d), social pedagogue (m/f/d) or nursing specialist (m/f/d)
• Further training as a case manager (m/f/d) desirable
• Strong communication skills and a confident manner
• Empathy, friendliness and open-mindedness towards our patients and relatives
