Nurse – Specialized Outpatient Palliative Care (m/f/d)

full-time or part-time.

• With empathy, professional competence and organizational talent:
• Advice, guidance and support for patients and their relatives in the home environment or in inpatient facilities for the elderly and disabled
• Coordination of palliative care in close coordination with doctors, nursing services, homecare companies and other professional groups
• Recording and assessment of symptoms
• Documentation and administrative tasks

• Completed training as a nurse (health and nursing / nursing specialist) and palliative care training
• Experience in palliative or hospice care
• Empathy, strong communication skills, a sense of responsibility and a structured and independent way of working
• Willingness to participate in on-call and emergency service
• Safe handling of digital documentation systems and class B driving licence
• Legally required proof of immunization against measles
