Helios Klinik Schleswig GmbH is looking for several coordinators for specialized outpatient palliative care (m/f/d) full-time or part-time, permanent as soon as possible.

• Palliative Counseling
• Coordination of SAPV recordings
• First-time visits
• Advising the cooperating care teams
• Further development and networking

• Completed training as a nurse (m/f/d)
• Basic course in palliative care (160 hours) according to SGB V§39a and the willingness to undergo further education and training
• Professional experience in outpatient or inpatient palliative care
• Experience with crisis intervention and social counselling
• Independent and structured way of working as well as creativity, open-mindedness and resilience
• Sensitivity in dealing with patients and caregivers in the special situation of a hospital stay
• Legally required proof of immunization against measles
