As soon as possible, we are looking for a

Case Management Employee (m/f/d)

full-time or part-time as a parental leave replacement until 31.07.2028

• Planning, coordinating, optimising and controlling patient discharges as part of discharge/case management
• Organization of short-term care, day care, long-term care, intensive home care, ventilation shared apartment, outpatient care services, hospice, SAPV connection, rehabilitation clinics, aids
• Application for nursing services, severe disability, aids and rehabilitation as well as registration and coordination of acute geriatrics
• Care and rehabilitation counseling for patients and relatives and participation in team meetings in acute geriatrics and neurological early rehabilitation
• Daily evaluation of the orders/processes currently in progress
• Coordination of further care – scheduling with patients, doctors, relatives, payers and external providers

• Completed training as a medical assistant (m/f/d) or training in health and nursing (m/f/d), case manager (m/f/d) or social worker (m/f/d)
• Experience in the organization in the dismissal process, social counseling or similar processes
• Confident handling of MS Office, HIS, telephone calls, GERDA
• Quick comprehension, commitment, motivation as well as ideas and innovative spirit for the optimization of processes
• Excellent communication skills, team spirit and willingness to cooperate with all interfaces of the hospital
• Motivation, flexibility and organizational skills even in stressful situations
• Legally required proof of immunization against measles
