Team Leader Case Management (m/f/d)

• Management of employees and technical responsibility for case management at HSK Wiesbaden
• Collaboration in projects to improve the quality of care
• Professional and personal development of subordinate case managers
• Determination of joint treatment paths with medical colleagues and coordination of patient/family discussions
• Active support, preparation and follow-up of doctor's rounds
• Case management and coordination of the diagnostic and therapeutic components of the patients
• Support of medical documentation/doctor's letter preparation
• Coordination of discharge management and support of case coding

• Successfully completed training as a nurse and experience in the field of case management
• Specialization, further education or studies in the field of nursing, nursing science, quality management or comparable fields of competence
• High level of professional and social competence
• Distinctive, independent and structured way of working
• Empathy, professional communication skills and enthusiasm
• Legally required proof of immunization against measles
