We are looking for you at the Leipzig Heart Center at the earliest possible date as a

Heart Failure Nurse / Healthcare and Nursing Assistant / Healthcare and Nursing Specialist / Nursing Assistant / MFA / Medical Assistant (m/f/d)

full-time or part-time.

• You will be responsible for coordinating care plans for patients with heart failure in close cooperation with the doctors and nursing staff
• You will be responsible for organizing and conducting training for patients and their families
• You will be responsible for monitoring compliance with treatment plans and medication management
• You will be responsible for documenting and reporting on patient progress
• Participate in interdisciplinary team meetings and actively contribute to the further development of treatment processes
• You will be responsible for developing and implementing strategies to improve patient care

• You have completed vocational training in nursing or a related field and several years of experience in the care of heart failure patients or in cardiology would be an advantage
• You have the additional qualification of heart failure nurse / heart failure assistant or are willing to complete this further training
• You have excellent communication skills and a high degree of empathy
• You have the ability to work in a dynamic, interdisciplinary team and have sound knowledge in the administration and documentation of care plans
• Your strengths include attention to detail and excellent organizational skills
• Legally required proof of immunization against measles
