We are looking for you as a Case Manager (m/f/d) for our Helios Clinic in Titisee-Neustadt at the earliest possible date

• Conception, planning and implementation of care quality audits in defined areas of responsibility

• Implementation and execution of nursing rounds
• Patient counseling and patient education
• Defining joint treatment paths with medical colleagues and coordinating discussions with patients/relatives
• Case management and coordination of diagnostic and therapeutic components for patients
• Coordinating discharge management and supporting case coding

• Successfully completed training as a nursing specialist (m/f/d)

• Specialization, further training or studies in the field of nursing,
• Nursing science, quality management or comparable fields of competence
• High level of professional and social competence
• Distinctly independent and structured way of working.
• Empathy, professional communication skills and enthusiasm
