At the location of the Helios Specialist Clinic Vogelsang-Gommern, we are looking for a

Employee in discharge management (m/f/d)

• Experience structured, plannable work in an elective clinic without an emergency room
• Support the therapeutic team and ensure high-quality work together with all professional groups
• Take over the task of determining the need for help after hospital discharge
• Take over or support you in applying for and organising a wide variety of forms of follow-up care (e.g. rehabilitation and the provision of aids)

• Completed vocational training as a nurse (m/f/d) or comparable qualification
• Empathetic and empathetic approach to our patients
• Enthusiasm for new things, team spirit and a cooperative attitude
• Willingness to develop further and participate in further training
• Legally required proof of immunization against measles
