To strengthen our team at the Helios Clinic in Pirna, we are looking for you as a nurse with specialist training in geriatrics (m/f/d) with immediate effect

• Conducting patient interviews (admission assessment)
• Discussions with relatives (clarification of the supply of medical aids, discussion of further care after discharge)
• Discharge management
• Consultations about care options with patients and relatives
• Support with the application for severe disability (severely disabled person application) and care level upgrade according to the file situation
• Application for follow-up treatment (AHB)

• State professional license as a nurse (m/f/d) or equivalent
• Professional experience or knowledge in the field of geriatrics
• Preferably specialist training / Zercur in the field of geriatrics desirable
• Interest in working with older people
• Good communication skills and teamwork as well as ability to convey complex information in a clear and understandable way
• Legally required proof of immunization against measles
