At Helios Klinikum Duisburg, we offer you a position as soon as possible and full-time or part-time as a Case Manager (m/f/d).

• Clarification and coordination of patient-specific care needs upon admission, peri-inpatient and discharge
• Initiation of measures and monitoring
• Case-related control and control of the treatment process
• Advice, training and support of the nursing and medical services with regard to the framework agreement on discharge management and other legal requirements
• Participation in team meetings, rounds
• Document quality review and optimization
• Interfaces and communication management with all stakeholders and service providers involved

• Completed training as a nurse (m/f/d) as well as experience in project management and/or management positions
• Ideally, an additional qualification, such as further training in case management, specialist training or a bachelor's degree in a nursing-related course of study
• Willingness to take responsibility and make independent decisions
• Competence in the development, moderation and implementation of solutions for complex issues
• Professional and forward-looking communication skills, both in writing and over the phone, with all interfaces
• Advanced computer skills, including experience with Microsoft Excel
• Strong skills in patient observation and analysis, strong social skills as well as independence and self-organization
• Legally required proof of immunization against measles
