To strengthen our practice team in the Medical Care Center in Bad Saarow, we are looking for you as soon as possible as a

Diabetes consultant (m/f/d)

full-time or part-time (min. 16 h / week)

• You will be responsible for coordinating our diabetology consultation hours
• After consultation with the attending physician, you will independently carry out and evaluate individualized consultations and training courses in accordance with current guidelines and evidence-based data
• The area of responsibility includes the implementation of structured training in the field of disease management programs for patients with diabetes mellitus type 1 and 2
• You will support the appropriate implementation of medically prescribed therapy and promote patient self-management
• You will also support the selection of remedies and aids for the care of people with diabetes mellitus
• You guide people with diabetes mellitus or their relatives in the documentation of therapeutic measures and check them

You have successfully completed further training as a DDG diabetes consultant (m/f/d), preferably with a medical training background

Preferably you already have previous experience with structured training modules and AID systems

A willingness to take responsibility, empathy and the ability to work in a team in the spirit of empowerment are not foreign to you

You also have an open and friendly demeanor, commitment and enthusiasm
