Medical Assistant Social Service / Patient Service Center (m/f/d)

To strengthen our team in the Patient Service Center, we are looking for you as a Medical Assistant / MFA (m/f/d) as soon as possible.

• Planning and coordination of transition management in the context of discharge management
• Close cooperation with internal interfaces (doctors and nurses) as well as with external providers of nursing, care and rehabilitation facilities
• Assistance and advice for our patients in personal emergencies
• Referral of our patients to short-term care, retirement homes and hospices
• Advice and application within the framework of social legislation
• Close cooperation with family care

• Completed training as a medical assistant / MFA (m/f/d)
• Experience in discharge/transition management
• Good knowledge of social legislation SGB V and SGB XI
• Good knowledge and experience in conducting conversations and advising patients and relatives
• Commitment, confident appearance
• High empathy and pedagogical skills
• Good IT skills
• Legally required proof of immunization against measles
